Biomarkers, which are objectively measurable characteristics of biological processes, are crucial for diagnosing and monitoring diseases. While widely used in other medical fields—such as hemoglobin A1C for diabetes or cholesterol levels for cardiovascular risk—scientifically valid biomarkers for psychiatric conditions are largely absent, with the exception of certain proteins for Alzheimer's disease. This gap exists because the biological basis of mental disorders is complex, subtle, and influenced by significant environmental factors like family history and trauma, making it challenging to isolate purely biological contributions.
Furthermore, psychiatric disorders are often conceptualized as behavioral syndromes—overlapping constellations of symptoms—rather than discrete illnesses. This clustering, due to shared genetic and environmental factors, complicates the search for single biomarkers that can accurately reflect the diverse mechanisms underlying these conditions. Historically, syndromes like tuberculosis and cancer were recognized before their biological causes were fully understood, a stage researchers are still working towards for mental illnesses.
Despite these challenges, meaningful advances in neurobiology have occurred since the DSM-5. These include the development of blood tests for Alzheimer's disease biomarkers like amyloid and tau proteins, the implication of immune factors such as C-reactive protein in mood disorders, and the identification of genetic risk factors that can be aggregated into polygenic risk scores. These developments pave the way for integrating genetic and psychosocial factors into a precision medicine approach for psychiatry.
Looking ahead, a 2013 survey predicted that the DSM-6 would incorporate laboratory tests for psychiatric diagnoses, a prediction researchers are now actively working to fulfill.
