Pilot programme to bring home care to patients after discharge from Ng Teng Fong General Hospital

A pilot programme is underway to bring home care to patients after they are discharged from hospital. It is an initiative by Ng Teng Fong General Hospital and social service agency, Loving Heart. It involves patients with complex chronic conditions who get help at home from retired nurses and trained community carers upon discharge. If successful, the pilot programme could eventually be scaled up across Singapore. Sabrina Ng reports.
