A woman, Jo Shaw, 35, was killed on May 3 in a suspected murder-suicide by her ex-partner, Ryan Kelly. Kelly forced his way into her Bristol home and detonated an explosive device. The Independent Office for Police Conduct (IOPC) will examine whether officers did enough to protect her.
