Donald Trump's recent executive order concerning vaccine schedules is generating confusion and doubt, according to public health experts, despite not altering current official recommendations from the Centers for Disease Control and Prevention (CDC).
Experts like Noele Nelson, a public health professor at Cornell University, state that the order "adds unnecessary confusion" and "undermines trust in vaccines without any change in the scientific evidence." Vincent Iannelli, a pediatrician and founder of Vaxopedia, echoed these concerns, predicting that the president's influence and "inaccurate comments" could lead to more people being "confused and scared," potentially resulting in delayed vaccinations for children.
The executive order reportedly attempts to reinstate sweeping changes to the vaccine schedule previously made by Robert F. Kennedy Jr., then secretary of Health and Human Services, which were later halted by a federal judge. However, an executive order can only offer guidance and does not change laws or set official recommendations. The CDC's guidance remains unchanged.
Wendy Parmet, faculty co-director at Northeastern University's Center for Health Policy and Law, noted that Trump's rhetoric, including disproven links between vaccines and autism, can have significant effects. She explained that while the president cannot legally alter recommendations or tell doctors to space out doses, his statements can prompt parents to request such changes, potentially leading to more doctor visits that insurance may not cover and increasing the risk of exposure to illnesses.
Nelson emphasized that vaccine spacing is based on decades of evidence to provide the best immune response and is not arbitrary. Iannelli added that Trump's rhetoric could also increase stigma around autism, potentially leading families to seek dangerous treatments.
Furthermore, Trump invoked religious freedom, suggesting the order might pressure states to reduce vaccination mandates for school attendance. While most states already offer religious exemptions, the administration's focus on "parental authority" could impact states that do not. Parmet anticipates legal challenges, citing ongoing litigation in states like New York, California, and Connecticut that have upheld vaccine requirements.
Iannelli observed a cyclical pattern where public concern about vaccines wanes as diseases are controlled, leading to lower immunization rates and subsequent disease resurgence. He shared an anecdote about parents initially hesitant to vaccinate their child due to online misinformation but agreeing to proceed after discussing the need for timely protection against diseases like tetanus and measles.
Pediatricians, Iannelli advised, should use "motivational interviewing" to address parental concerns, listen to their worries, and help them find reasons to vaccinate, rather than lecturing or dismissing them. He stressed that addressing these concerns, amplified by misinformation, can be done efficiently and effectively.
