Brearey, who led a group of seven consultants with similar concerns, said he "naively" assumed senior managers would act on their suspicions. Hospital bosses conducted internal reviews in 2015 and 2016, but Cheshire Police were not involved until May 2017. Letby remained employed until her arrest in July 2018. The Thirlwall Inquiry, which investigated the events, found a "toxic negativity" within the NHS that discouraged staff from speaking out and made 17 recommendations, including CCTV for all cots and incubators in neonatal units. The inquiry was critical of former hospital leaders Tony Chambers, Alison Kelly, and Ian Harvey. The hospital trust has stated it is now a different organization with improved governance and a more open culture.
